How many people are suffering with medication reconciliation? Does anyone out there have a good practice in place?
I am sure many of us would like to get some ansers on this very confusing subject. Reconciliation has to take place on admission, any transfer to a different level of care and at discharge. That is the easy part to understand from there the waters get muddy.
