I would think (and I'm not a dietician, so I'm not certain) - but protein and fats would be the things you are missing the most. You are missing everything from what you are saying, but those are really taking the toll.
Really try keeping some protein shakes or bars around the house that you can use through-out the day. As opposed to someone using these to lose weight, however - you'll use these to supplement other meals.
Sunken eyes tend to occur when a body is in starvation mode - so you really are not getting enough fat. Fat gets a horrible rap - people think it is the worst thing in the world, but the fact is, the body does need
some. It's just that in the case of most people, we get way more than enough.
If you are going to take vitamin supplements, the big ones would be: B12, C, Iron, & Calcium Citrate, plus a multi-vitamin. If you really can't handle taking all those different ones, I'd reccomend one that is formulated for gastric bypass patients called "Optisource" - it's a chewable multi-vitamin with extra of the above things I mentioned (because people who have had bypass surgery have trouble getting enough of these vitamins). You can usually find it at Walgreens.
Milk is a big thing - and in your case, I'd go ahead and recommend drinking whole milk.
But I honestly think the best thing you can do for yourself is to get to a dietician or nutritionist, or maybe even an endocrinologist. Your "cosmetic" problems sound more like the symptoms of something, rather than the actual problem. And it would be better to find out the root source of your weight/eating issues, before it starts to affect more than your appearance - like you heart.
