..something you shouldn't have to do with someone who is fulfilling a critical function to your health. You don't want to be pregnant - EVER - and since birth control has a certain failure rate she is putting you at risk of developing a condition (pregnancy) that is unacceptable to you.
Have you tried contacting Planned Parenthood about a referral? You can still keep the nimrod lady doctor for regular checkups but get the procedure done by someone who is not obstructionist about your desire for permanent birth control.
