Jan: It's not up to me to determine when BF is best or not. That's a decision that should be left entirely to the mom and partner, with the advice of mom's medical practitioner of choice.

Typical "no-no" situations would exist if mom was a drug/alcohol abuser, HIV positive, taking chemo/radiation, taking Lithium or other drugs that can build to toxic levels in the child.

Mom might also rethink the issue if her health could be impacted in any way: say mom was severely malnourished, had some long-term debilitating illness (I know several mothers with Rheumatoid Arthritis and MS and other illnesses/diseases/conditions that chose not to BF because it was very taxing to them), or had to choose between taking medication to treat an illness/disease/condition and being able to adequately care for their child or not taking the medication and having problems that could impact their ability to care for the child.

Providing information and support and helping moms weigh options is what lactation consultants, in my book, should focus on.

Thankfully I've learned how to pick a pediatrician and i've never had a problem. The only thing our doc has ever said it "dont beat yourself up if you need to supplement", this was with my son who has alpha1 anti-trypsin deficiency and must eat an extremely high caloric intake daily. Try as I did I couldn't get my milk to be everything he needed, but it doesn't mean I stopped nursing...I continued with the help and support of the pediatrician, a dietician, several specialists and the LC.

The doc I used in orlando had an entire lending library for nursing moms, a special room they could go in to weigh their baby at any time free of charge, a LC on staff...they were great.

The doc I go to here in GA doesn't have all of those extras but he's just as supportive. I didnt really ask him much about it since I've nursed 3 of my 4 kids into toddlerhood.

If a doc of their staff wasn't openly supportive of my nursing my kids i'd find another doctor. I wouldnt make a big deal over it.

Check out this blog - written by another pediatrician. I think Dr. Parker well states the issues and in particular has an interesting perspective on parents contributing to the situation in his last paragraph.

I wouldn't hire him as my child's pediatrician but I like the next to last paragraph.
