How would it work if you cut an old Tee shirt front (one with a cute picture) and iron it on a new Tee? I guess you could sew it on and perhaps do some quilting to keep it flat on the new shirt or would it be too heavy on the front of the shirt?

It should be relatively easy to do. I would use a light-weight fusible iron-on interfacing cut to the same size as the old t-shirt picture. I would be sure to use a press cloth to try and not distort the picture. The edges of the picture could be zig-zagged with a close stitch to encase the edges.
